The Barwon Primary Care Forum has now formally merged with the G21 Health & Wellbeing Pillar .
The G21 Health & Wellbeing Pillar is a voluntary alliance of over fifty community and health services across five local government areas: City of Greater Geelong, Surf Coast Shire, Colac-Otway Shire, Golden Plains and Borough of Queenscliffe.

The G21 Health & Wellbeing Pillar is funded under the State Government Primary Care Partnership Strategy . The strategy aims to improve the overall health and wellbeing of Victorians by:
- Improving the experience and outcomes for people who use primary care services.
- Reducing the preventable use of hospital, medical and residential services through a greater emphasis on health promotion programs and by responding to the early signs of disease and/or people's need for support.
The activities of the G21 Health & Wellbeing Pillar are grouped around four key deliverables:
- Partnerships
- Integrated Health Promotion
- Service Coordination
- Integrated Chronic Disease Management
